Immobilization of the head, neck, and spine is done to prevent a patient with a possible neck or spine injury from moving and potentially hurting themselves further. In the vast majority of cases, immobilization is done in a pre-hospital setting by EMS professionals, usually at the site of the injury or accident. On-site emergency medical personnel will make the initial assessment of the patient and take into account the cause of their injury and whether or not it creates suspicion of a head or spinal injury. The patient’s overall mental state is also a major factor in considering immobilization, as well as their neurologic deficit.
Patients who should have spinal immobilization include the following: those suffering from blunt trauma, those who report spinal tenderness or pain, patients with an altered level of consciousness, patients with neurological deficits, patients who exhibit an obvious anatomical deformity of the spine, or patients with trauma who are also intoxicated from alcohol or drugs.
A cervical spine that is positioned and maintained in neutral alignment should preserve the space within the spinal canal that normally surrounds the spinal cord. Even the smallest deviation from the neutral alignment can decrease the diameter of the spinal canal and the space available for the spinal cord. This can lead to pinching or compression of the spinal cord and unnecessary injury.
In pediatric patients, cervical spine immobilization is preferred over full spinal immobilization unless signs or symptoms of other spinal injuries are present. Some of the reasons for head immobilization in pediatric patients include the following: neck pain, altered limb neurology that can’t be explained by trauma, muscle spasms of the neck, a low score on the Glasgow Coma Scale (GCS), or high-risk trauma such as a high velocity motor vehicle accident or a significant upper body injury.
